n engl j med 370;6 nejm.org february 6, 2014 582 are highlighted. First, we concentrate on serodiscordant couples in long-term partnerships (the target population of the HIV Prevention Trials Network 052 study); Hontelez and de Vlas simulate a generalized epidemic. Second, we focus on patients who are already identified and in care (treatment as prevention). In contrast, Hontelez and de Vlas describe an analysis of a test-andtreat strategy in which a patient’s choice not to undergo HIV testing, an inadequate linkage to care, and a lack of initiation of ART would all attenuate the intervention effect. We respectfully disagree with the suggestion that by restricting attention to firstand secondorder transmissions we failed to account for a substantial number of downstream infections. As noted in Table 2 of our article and in Figure S2 in the Supplementary Appendix (available with the full text of our article at NEJM.org), even second-order infections had no material effect on our findings over a period of 5 years. Projections beyond second-order infections in serodiscordant couples require assumptions about the future (e.g., ART efficacy, loss to follow-up, and sexual behavior) that are well beyond the trial data and unnecessary to reach our critical policy conclusions.

Ischemic preconditioning or heat shock pretreatment ameliorates neuronal apoptosis following hypothermic circulatory arrest.

OBJECTIVE Hypothermic circulatory arrest has been widely used in complex cardiac and aortic surgery. Stroke and/or neurologic injury can occur after prolonged hypothermic circulatory arrest, possibly due to apoptosis. Ischemic preconditioning has been widely used as a neuroprotective tool, but its application in neuronal injury under hypothermic circulatory arrest has never been studied. METH...

Scandinavian SSAI clinical practice guideline on choice of first‐line vasopressor for patients with acute circulatory failure

BACKGROUND Adult critically ill patients often suffer from acute circulatory failure, necessitating use of vasopressor therapy. The aim of the Scandinavian Society of Anaesthesiology and Intensive Care Medicine (SSAI) task force for Acute Circulatory Failure was to present clinically relevant, evidence-based treatment recommendations on this topic. METHODS This guideline was developed accordi...

Physiology of blood flow and oxygen utilization by peripheral tissue in circulatory shock.

The primary defect that characterizes circulatory shock is acute perfusion failure, in which oxygen metabolism is critically impaired by decreased delivery of oxygen to tissues. Four categories of hemodynamic deficits are described as the basic mechanisms of circulatory shock: hypovolemia, cardiac failure, distributive deficits, and vascular obstruction. Perfusion failure can be identified by t...

A Practical Approach to Mechanical Circulatory Support in Patients Undergoing Percutaneous Coronary Intervention: An Interventional Perspective.

Percutaneous mechanical circulatory support has been used to stabilize patients in cardiogenic shock and provide hemodynamic support during high-risk percutaneous coronary interventions for several decades. The goal of this paper is to provide a practical approach to percutaneous mechanical circulatory support in patients undergoing percutaneous coronary intervention with cardiogenic shock and/...
